The following nursing facilities have appeared on all four of our Nursing Home Watch Lists published between 2000 and 2005 as well as on the list of homes to avoid in the current Consumer Reports Nursing Home Quality Monitor.

For the first four lists we published, we examined state inspection surveys for deficiencies that in our judgment raised questions about the quality of care delivered to residents. For our Nursing Home Quality Monitor, we examined staffing levels and quality indicators as well as deficiencies reported on state inspection surveys. We also note whether the state had fined these nursing homes during the period 1999-2004. Eight of these homes received no fines during that period.

We recommend that you avoid placing a family member in one of these facilities. If a relative already lives in one of these homes, you need to be extra vigilant about the care that person receives. Visit often and stay involved in the care planning. And finally, speak up if you notice poor care. Report any concerns to your local long-term-care ombudsman and state regulatory agency.
